NEW PATIENTS are welcome to book online.
You’ve got choices! 90 and 60 min visits are available for both New and Returning Patients.
New Patient Pro Tip: because my schedule books up several weeks in advance I recommend booking both a New Patient visit and 1-2 Returning Patient visits, either 60 or 90 min in length. It’s easier to cancel than to finds spots when the schedule gets full. Cheers!
To conveniently complete your New Patient Intake Forms electronically AND have your health insurance verified BEFORE your first visit.
Here's what we need from you:
In your Schedulicity Profile, please include/update your Date of Birth. With this information we can send you an invitation to complete all necessary paperwork electronically through our ChARM Electronic Health Records system.
Yippee, no need to come 15 minutes early to complete paper forms :)!
Then watch for emails from Cultivate Wellness (Patient Portal Registration and Questionnaire Notification).
Please complete these Intake Forms ASAP! Ideally 48 hrs before your visit.
This will provide us with all we need to verify your insurance benefits and be best prepared to provide you with excellent service.
If insurance benefits are not verified prior to your visit we will collect the cash rate.
We will credit you back once benefits have been confirmed.
