ALL Forms can now be done electronically before you arrive for your first appointment.
In order to receive your New Patient Forms we need your Date of Birth.
Please do one of the following:
include these details in your Schedulicity Profile when booking online

If you plan to use your Health Insurance...
*Ideally you complete and submit your Forms 48 hours PRIOR to your first appointment so we can have time to verify your benefits before you arrive. If benefits are not confirmed before your visit, we will collect the cash rate and happily credit you back once benefits are verified.
